OSHC Co-curricular and Events Notification Form

- I give permission for my daughter to attend her Co-curricular activities on Pymble campus.
- I agree to resubmit this form if my child's activity times change.
- I understand my child will be unable to attend activities if I have not shared updated information with the OSHC Centre.
- (Years K- 2 only) I accept that my child will be attending and travelling to and from activities during an OSHC session and will be under the supervision of school walkers organised by Pymble Sport. I have also notified Prep Administration for the purpose of organising travel arrangements for my child. OR (Years 3-6 only) I accept that my child will be attending and travelling to and from activities during an OSHC session and will not be under the supervision of the Pymble LC OSHC staff.
- When my child is not returning to the OSHC Centre after the activity, I have notified OSHC administration and I authorise an OSHC staff member to sign my child out from the OSHC Centre.
- I give permission for my child to attend regular excursions within the grounds of Pymble Ladies' College when accompanied by OSHC Educators.
- I give permission for my child to participate in regular rehearsals for Emergency Lockdown and Evacuation procedures.
